Barbara Vale is a suburban housewife who desires a happy home life, but has an extremely dysfunctional family. Her husband Vincent is an abusive drug addict, her daughter Ruthie has affairs with married men (her current one with her gym teacher), and her son Fletcher kills and skins neighborhood pets. |

Barbara stoically endures this until a milkman breaks into her home and rapes her, causing a drastic and more aggressive change in her personality. This trend reaches its head when Barbara discovers Fletcher's workshop and tries to tell Vincent, only to have him nearly kill her in the midst of a drug-fueled rage. Fletcher calls the police, only to have Vincent convince the responding officer to leave. The next morning, Barbara smashes her television in a rage, only to be confronted by the ghosts image of a 1950's sitcom housewife named Carol White. Carol encourages Barbara to "discipline" her family. |

That evening, gruesomely murders each of her family members before traveling to the city to confront the milkman. Securing a gun and killing a group of drug addicts she asks for directions from, she finds the milkman in a dilapidated dairy plant. He lectures Barbara and triggers her memories of his attack, causing her to faint and awaken in a tank of rotten milk. Carol frees Barbara and motivates her into challenging the milkman again, this confrontation resulting in Barbara killing him and finding a map with various locations marked. |
