Maryland Route 18 is a Maryland state highway that runs for 20 miles (32 km) through Queen Anne's County, Maryland, and for much of its length it is known as Main Street, serving as the Main Street for the towns it connects. Route 18 is also part of the Chesapeake Country National Scenic Byway.

Route 18 used to be one of the main east–west roads in Queen Anne's County prior to the routing of U.S. Route 50 onto the Eastern Shore. It now serves mainly local traffic, however due to increasing backups on Route 50, it has attracted motorists as an alternate route to reach the Chesapeake Bay Bridge, although the Maryland State Highway Administration discourages the practice with signs indicating that Bay Bridge-bound traffic should stay on US 50.

At its intersection with Maryland Route 8, Route 18 turns north onto Business Parkway and continues to Love Point [1]. The other end of the highway is at an intersection with Maryland Route 213 in Centreville.

There is no route that is officially numbered MD 18; the route that is signed as MD 18 is made up of MD 18A, MD 18B, MD 18S, and MD 18C.
