Maryland Route 18 runs for 20 miles through Queen Anne's County, Maryland, and for much of its length it is known as Main Street, serving as the Main Street for the towns it connects. Route 18 is also part of the Chesapeake Country National Scenic Byway.

Route 18 used to be one of the main east-west roads in Queen Anne's County prior to the routing of U.S. Route 50 onto the Eastern Shore.
