- Ambivalent. There are about three English-language news sources based in Hong Kong which qualify as reliable, none of which seem to have anything resembling a style guideline. They do tend towards capitalizing or using "South Island MTR line" etc.; over 90 percent of "Island Line" or "South Island Line" uses in the South China Morning Post are capitalized and there are no uses of "Island MTR line". About 90 percent of "East Rail Line" uses in news articles have all three words capitalized (even though MTRC just tacked on "Line" to the KCR's "East Rail"); only one news article (also SCMP) uses "East Rail MTR line". - There's no percentage threshold in the guideline but I think it could be somewhat different for mainland China since English isn't an official language and there are even fewer local English-language publications to draw from.
