Paweł Jakubiak (born on December 28, 1974 in Poznań, Wielkopolskie) is a field hockey defender from Poland. He was a member of the Men's National Team that ended up in twelfth and last place at the 2000 Summer Olympics which was held in Sydney. Surprisingly the team was qualified for the game by defeating big team like New Zealand at the 2000 Men's Field Hockey Olympic Qualifier in Osaka, Japan.
