Plot

Before my edit the plot section was incomplete. It essentially described the opening of the novel and then had just a few sentences that abstracted the rest of the plot. I've fleshed it out a lot (perhaps a bit too much not sure). I've also provided references for an anecdote about Billy Wilder and moved that anecdote from the plot section to a background section which I also added. One thing to do is I don't currently have a copy of the book at hand. I'm going from memory but I've read the book several times and just re-read it a short time ago. The only thing is I can't remember the names of the doctor or the gangster so I've left them out for now but will add them as soon as I can get a copy of the book.
