Maryland Route 18 runs for 20 miles through Queen Anne's County, Maryland, and for much of its length it is known as Main Street, serving as the Main Street for the towns it connects. Route 18 is also part of the Chesapeake Country National Scenic Byway.

Route 18 used to be one of the main east-west roads in Queen Anne's County prior to the routing of U.S. Route 50 onto the Eastern Shore. It now serves mainly local traffic, however due to increasing backups on Route 50, it has attracted motorists as an alternate route to reach the Chesapeake Bay Bridge, although the Maryland State Highway Administration discourages the practice with signs indicating that Bay Bridge-bound traffic should stay on US 50.

Looking at its intersection with Maryland Route 8, it may appear that Route 18 ends there, however it actually turns north onto Business Parkway and continues to Love Point, while Route 8 begins here and goes south. The other end of the highway is at an intersection with Maryland Route 213 in Centreville.
